Taxi Medicals
insurance forms
holiday cancellation forms
medical reports
fitness to travel certificates
private prescriptions
passport signing
some vaccination services
You can find our fees for these services in the surgery.
Please note if you require any of the above you need to contact reception on 01782 593344 please do not book an online appointment as some of these if appointments required will require longer than the standard 10 minutes and you may find if you do book and attend you will have to rebook in to enable the full consultation to take place.
