If you are travelling abroad please make sure you contact us in plenty of time to arrange any vaccinations that may be necessary. To help the travel nurses assess your travel needs it is important that they are in receipt of the attached travel risk assessment form before your appointment. Once this assessment form has been completed and received, your form will be passed to the practice nurse to review and decide which vaccines are required and will be in contact in due course. Some vaccines require payment and if this is the case payment will be required upfront for all vaccinations, as we have to order them in to the surgery. A receipt will be given as proof of purchase. You will then be notified once your vaccinations arrive at the practice and an appointment will be arranged.
