If you’re aged between 40 and 74 and haven’t already been diagnosed with heart disease, diabetes, kidney disease, stroke or high blood pressure, you may be eligible for a health check.
If you are eligible an invitation will be sent out, but please contact the reception team, if you haven’t taken up the offer and would like to book in for a 20 – 30 minute health assessment with either our Health Care Support Worker or practice Nurse.
At your health check you will be asked about your lifestyle and family medical history. You’ll also have some routine tests like height and weight, blood pressure and cholesterol. You will be able to discuss your results with your health check provider and talk about what you can do to be healthy.
Taking up the offer of a free NHS Health Check will improve the health and wellbeing of people in Stoke & Staffordshire and in the long term will reduce the cost on health and social care services.
